Hospices in eastern ontario - INTERACTIVE MAP

HOSPICE:  a place where physical, emotional and spiritual supports are provided in a peaceful homelike setting, with a focus on pain and symptom management and quality of life. 

HOSPICE WITH BEDS:  a hospice that has beds where a person can stay.

PALLIATIVE CARE UNIT:  The palliative care unit at the Élisabeth Bruyère Hospital in Ottawa provides care to patients and families requiring in-hospital care. Patients may be admitted for a short period of time to manage difficult pain and symptoms, and will be discharged once these needs are met. Other patients may be admitted for care at the end-of-life when their care needs cannot be met in other settings.

It is important for patients to connect with a Home and Community Care Coordinator.  Patients will have access to more services with a Care Coordinator who can help them develop a care plan that is right for them.
